How to Register with the Surgery

If you live within our practice area we are happy to register you.

To register, bring your medical card to the surgery or fill in a GMS1 form, available from the surgery or downloadable HERE

We will make you an appointment with a member of the practice team to take some details and complete the registration.
 

You can express a preference for registration with a particular doctor.

All patients at the Family Medical Centre can see any of the doctors practicing there.
